- "Signed by order and in behalf of the Congress, John Hancock president. Attest. Charles Thompson [i.e., Thomson], secretary."
- Two states noted. In the first (Bristol B4407, Walsh 14) Hancock is misspelled 'Hacock.' Printed area measures 39.4 x 29.2 cm. In the second state (Bristol B4408, Walsh 15), Hancock's name is spelled correctly, and the last three lines have been reset.
- Probable imprint determined by Frederick R. Goff, whose findings are detailed in the Library of Congress Quarterly journal of current acquisitions, v. 5, no. 1, November 1957, p. 12-16.
- Text in two columns, with fifty-eight lines in the first.
