Sample survey forms to evaluate the Jefferson City, Missouri, gifted and talented programs are presented. Intended to assist in planning and development of future programs, separate surveys are addressed to teachers, secondary students, administrators, elementary students, and parents. Surveys request responses describing attitudes toward specific statements. The final document in the series presents survey directions and a scoring manual. Field testing information indicates that the statewide sample for the attitudinal surveys were 615 elementary students (grades 5 and 6); 262 secondary students; 547 parents or students identified as gifted and/or talented; and 169 teachers and 44 administrators of gifted/talented students. Tables list correlation matrices for five factors (attitudes toward program, improved career awareness, perception of peer attitudes, improved self awareness, and variety of activities). Answer sheets and scoring templates are appended. (CL)
