- Authorship of the Eikon basilike was originally attributed to Charles I, but according to Madan (pp. 125-33) it was written by John Gauden who probably included some authentic writings of the king.
- Title page differs from that of Madan no. 11 by having broken rules below title, between epigraphs, and above imprint; "solitudes" and sufferings" are printed with swash "u"s; chapter 1 heading begins with "Upon" rather than "Vpon", the "U" being swash; p. 64 includes the typo: "oppressive faces" for "oppressive fears"; G2r has catchword "Militia,".
- Frontispiece is woodcut of the King's execution with caption below: "The martyrdom of King Charles ye first"; not listed in Madan.
- With contents on 3 pp. at end.
