This facilitator's guide is one of three documents recommended for use in the development of a local level linkage system for the referral of severely handicapped students from education to rehabilitation. Introductory materials include a discussion of right versus entitlement and overview of the facilitator's role. The first section provides a checklist of the facilitator's activities for developing a referral-based local level linkage system and a timeline of linkage development activities. The second section contains an example of the development process used in Project INTERACT's Model Linkage System for the Referral of Severely Handicapped Students from Education to Rehabilitation. A completed copy of the checklist (provided in the previous section) is given. Other contents include sample agency interview questions; charts depicting the primary interagency communication network; model linkage system flow charts reflecting the educational continuum existing in Franklin County, Ohio, and points of contact established to create a linkage system; and model procedures for the referral of severely handicapped students from education to rehabilitation. These procedures address eligibility criteria for student referral, considerations for referral, referral information packet, questions related to rehabilitation plan development, referral and consultation process, and referral timeline.
